What you can make a referral for
You can ask for help with care and support for yourself or another Camden resident. This includes:
- Occupational Therapy
- sensory needs services
- hearing or visual impairments
- physical disabilities
- personal care and day-to-day living
- providing unpaid care for a friend or relative
Safeguarding
If you have safeguarding concerns for a Camden resident who may be vulnerable to abuse or neglect, you should not use this form.
To report safeguarding concerns, call 0207 974 4444 (Monday to Friday, 9am to 5pm) and when prompted say "Safeguarding". If someone is in immediate danger, call 999.
This referral form is also not for mental health, housing or learning disabilities support:

Before you make a referral for someone else
If you’re making a referral for someone else, there are 2 steps you should take first.
Check they’re a Camden resident
We can only accept referrals for Camden residents. Use the GOV.UK postcode checker to check which local council the person you want to refer lives in.
Get their details
If you’re referring someone else, you need to know their:
- contact details
- date of birth
- address
How to make a referral
When making a referral, you should answer the questions in as much detail as you can.
If you need help filling the form in, call 0207 974 4444 (Monday to Friday, 9am to 5pm) and when prompted say "Adult Social Care".

What happens next
Your referral will be reviewed by the Adult Social Care team and we’ll get in touch with the person who has been referred within 5 working days. We’ll call from a withheld number.
If we cannot reach the person who has been referred, we'll try to contact their GP, the referrer or the alternative personal contact listed in the form.
